Public Health Nurse – Perry County Health Department:  Full-time position available.  Responsibilities include: Initiate Bureau of Children with Medical Handicaps Program services by providing home visits for children; develop, implement and evaluate family centered assessments and care plans; coordinate referrals to appropriate resources; develop and provide health education and guidance to families; assist with administration of immunizations and conducting communicable disease investigations; maintain accurate records and reports; participate in the event of any public health emergency. Minimum qualifications: must be licensed registered nurse pursuant to Section 4723.13 of the Revised Code and have a valid Ohio driver’s license with ongoing proof of auto insurance.  Send resume, cover letter, and references to Angela DeRolph, Health Commissioner, Perry County Health Department, 212 South Main Street, P.O.
