Vital Statistics Records
Ohio records of births/deaths are filed within the registration district where the event occurred. If the birth/death occurred within Perry County, the record is on file at the Perry County Health Department. If the birth/death occurred outside of Perry County, but within Ohio, the certificate can be obtained from the Ohio Department of Health or the county in which the event occurred.
Disclaimer: Please be aware that the local and state Vital Statistic Offices must comply with Federal regulations. As federal and state laws change, older birth certificates may have to be replaced as certain federal agencies will begin to refuse to accept these older certificates. Replacement certificates will be available at the prevailing rate for Certified Copies.
Certified birth/death certificates are available for $25.00 per copy from the Perry County Health Department for persons who were born or died in Perry County, Ohio after December, 1908.
Please note: Only birth/death certificates for Perry County, Ohio are available from our department. Birth/death certificates for other counties must be obtained from those counties or from the Ohio Department of Health (see additional information below.)
"Certified" birth/death certificates are required for most legal purposes. "Certified" birth/death certificates are available for $25.00 per copy. Uncertified birth/death certificates are no longer available in Ohio as of July 1, 2003.

Obtaining Vital Statistic Records Via Mail :
Alternatively, you may obtain copies of birth/death certificates by mail. If you would prefer to order a certified birth/death certificate by mail, please list the following information along with a self-addressed, legal-size stamped envelope with payment and mail to Perry County Health Department - Vital Statistics, 121 West Brown Street, P.O. Box 230, New Lexington, Ohio 43783.
1. Requester's name, address, phone number.
2. Type of record (birth/death) and number of certified copies desired.
3. Last name (at birth/death), first name, and middle initial of the desired birth/death certificate.
4. Date of birth/death of individual for whom the birth/death certificate is being requested.
5. Place of birth/death, if known, (Hospital name, home, etc.) including Ohio city/village/township information.
6. Parents names and, if known, the mothers maiden name.
Note: No certified birth/death certificate(s) will be mailed until payment is received - No out-of-state personal checks can be accepted and out of state payment should be made by a certified check or money order that is made out to the "Perry County Health Department."
