Can I get a Continuous Glucose Monitor through Perry?

Yes! We are actively prescribing Dexcom G7's to qualified members. The qualifications include being insulin treated and use of an iPhone so that the data from the sensor can stream to your Perry Health care team. If you are already prescribed CGM from your physician (Dexcom or Freestyle Libre 2 or 3) we can connect it to your account. If you're interested reach out to us at 469-772-9400.

How do I sign up for Perry Health?

If you are interested in Perry Health’s services, you can call us at 469-772-9400 and one of our Enrollment Specialists will ask you a few questions about your medical history to determine if you qualify. The process doesn’t take more than a few minutes, and the Enrollment Specialist can answer any questions you may have.

What happens after I sign up?

Your Enrollment Specialist will schedule you for your first clinical visit with one of ours doctors or nurse practitioners, who will further gather some additional information about your medical history and determine your testing cadence. This visit will conclude by scheduling a visit with your accountability coach and possibly various consultations with our clinical team and dietitians, who will help guide you on your journey to lower your A1c. You'll meet with your coach a couple times a month for 10-15 minutes, as well as have structured 30 minute meetings with our clinical and dietary team. We'll also ship out your cellular connected Blood Glucose Meter and strips and supplies will be regularly delivered to you. If your readings ever trigger an alert in our system, a member of the clinical team will reach out immediately. All of these visits will be covered with your monthly copay if you even have one!

Who is Perry for?

Perry is for anyone who is 26 years or older who is diagnosed with Prediabetes, Type 1 or Type 2 Diabetes.

Who is eligible to join Perry Health?

You are eligible for Perry Health if you are 26 years or older and diagnosed with Prediabetes, Type 1 or Type 2 and on a Medicare Part-B, an eligible Medicare Advantage Plan or select commercial insurance plans. To find out if you are eligible call us at 469-772-9400.

How much does Perry cost?

The cost of Perry Health will vary depending on your insurance. For people on Medicare Part-B, Medicare Advantage and certain Commercial plans, you can expect to pay up to $30 per month, though most of our users do not have a Co Pay. This includes the Perry Blood Glucose Meter, supplies shipped to your door or a CGM prescription if you qualify, and access to our clinical program. If you have a qualified supplemental insurance plan or are a Qualified Medicare Beneficiary, you likely won't have any out-of-pocket costs for Perry Health. Costs should not be a barrier to get the care you need. If you can't afford the service, call us to see if you qualify for Financial Assistance.

What type of Blood Glucose Meter does Perry send?

If you qualify for the program, we'll send you either a traditional blood glucose meter that requires a small drop of blood from your finger or, if you qualify, we can prescribe you a Dexcom G7 CGM. Our monitor is FDA approved and is connected through all major wireless networks. Your readings, both from the BGM and CGM will be instantly transmitted to our team with each reading you take. If you are already on a CGM such as a Dexcom G6, Dexcom G7, Freestyle Libre 2 or Freestyle Libre 3, there is a chance you can continue using your device with our program.

Do you prescribe medications?

At times, our doctors or clinical team will adjust your medications or doses while working in close contact with your Primary Care physician or Endocrinologist.

Does Perry accept insurance?

Yes. We accept Medicare Part-B and most Medicare Advantage and Commercial Insurance plans. Call us at 469-772-9400 and we can check your eligibility.

What can I eat on the program?

Your team of Dietitians will work with you to create a customized plan while you are on Perry. There is no one-size fits all approach to diet and nutrition, so our team customizes your plan based on your specific needs.

Where are you located?

While we were founded in New York City and call that our home base, Perry is a remote program. Our clinical team is licensed to operate in over 16 states. There are no in-person visits with Perry. All of your meetings with our team occur over the telephone or through video. You can also text your care team any time day or night.

How is Perry different than going to my doctor?

Because your readings are instantly transmitted to our team each time you take one, we are able to react to trends and alerts in the data in real time. The Perry program is made to supplement your in-person care, and to give you access to a team of diabetes experts around the clock, without having to wait for your next appointment.

What is the time committment?

This answer depends on your specific needs. Some members work with us as little as 20 minutes per month, while others can have multiple 30-minute sessions with our team. It depends entirely on your goals and desired outcomes from the program.

Do I need an app to use Perry?

No. All of your interactions with your care team will be via text message and phone.

What states is Perry live in?

We're live in 18 States. Alabama, Arizona, Colorado, Florida, Georgia, Idaho, Kentucky, Louisiana, Maryland, Mississippi, New Jersey, North Carolina, Ohio, Tennessee, Texas, Utah, Virginia, Wisconsin.

Does my doctor have to approve this?

Your in-person physician does not have to approve you for Perry Health. Our services are designed to supplement the in-person care you receive from your primary care provider or endocrinologist, not replace it. If you qualify for the program and you’re interested, you are able to enroll.

What certifications / qualifications do Perry's coaches have?

Our Accountability Coaches are all board-certified by the National Board of Health & Wellness Coaching.

Who are the doctors and clinical team at Perry?

Our program is led by Doctors Avinash Ravilla, DO and Maria Peña, MD. Our team of Registered Nurses and Registered Dietitians are all state licensed by the various state governing bodies and have a rigorous background in diabetes care and management. Our Accountability Coaches are all board-certified by the National Board of Health & Wellness Coaching.

I have a Medicare Advantage plan, can I join Perry Health?

Seniors 65yo and older, with Medicare Part-B or an eligible Medicare Advantage Plan are eligible to receive Perry Health's services at little to no out of pocket cost. Give us a call at (855) 525-1350 to learn more.
